You can request a ballot from the Clerk’s Office any of these ways:

1. Apply for a ballot online!
2. Complete an application (PDF) and email to the Clerk.
3. Complete an application (PDF) and mail to:

4. Call 517-694-2135 to have an application mailed to you.
